Chronic Disease Management

Reclaiming Your Health

At the Y, we empower people of all ages and abilities to change their lives for the better. Our suite of chronic disease prevention and management programs use an integrative approach that combines classroom sessions, peer support, and tailored physical activity to provide you with the support needed to adopt healthier habits to support improved quality of life.

You Can Take Control

In the YMCA’s Diabetes Prevention Program, a trained lifestyle coach will introduce topics in a supportive, small group environment and encourage participants as you explore how healthy eating, physical activity and behavior changes can benefit your health and delay or prevent the onset of type 2 diabetes. Small steps make a big difference and can help you live a longer, healthier life.

Reduce Your Symptoms

Pedaling a bicycle may change the life of someone with Parkinson’s disease. Research conducted at the Cleveland Clinic showed a 35% reduction in symptoms by the simple act of pedaling a bicycle at a rapid pace – optimally, 80-90 revolutions per minute. To participate in our Pedaling for Parkinson’s program, a medical clearance form and a referral from a medical provider are required.
